Prognosis

•Good to poor, depending on the severity of the corneal involvement (Fig. 7-9H). Chronic recurrent bouts of ocular inflammation are common. The prognosis of a corneal graft is guarded in severely neurotrophic corneas. Postherpetic neuralgia can be devastating in some patients.

INTERSTITIAL KERATITIS (SYPHILITIC, NONSYPHILITIC)

Interstitial keratitis is an uncommon, typically bilateral inflammation of the corneal stroma without primary involvement of the epithelium or endothelium. It has a wide variety of causes.

Etiology

•Congenital or acquired syphilis (Treponema pallidum)

•Lyme disease (Borrelia burgdorferi)

•Tuberculosis (Mycobacterium tuberculosis)

•Leprosy (Mycobacterium leprae)

•Cogan syndrome: tinnitus, vertigo, and deafness. Associated with polyarteritis nodosa, granulomatosis with polyangiitis (Wegener granulomatosis), and rheumatoid arthritis. Cogan syndrome is a rare condition.

•Herpes simplex, varicella zoster, Epstein-Barr, and mumps viruses can also cause interstitial keratitis.
